The Form Builder lets you create the wellness checks, RPE prompts, and custom questionnaires your athletes complete. This article covers where to find the builder, how to create a form, how to add questions, and how to assign it to athletes or a group.
Where to Find It
Log in with a coach or administrator account.
Navigate to Forms Module
The builder lists any existing forms and surveys. From here you can open one to edit or start a new one.
Create New
Select New Form
Give the form a clear name, for example "Morning Wellness" or "Session RPE".
Add a short description so athletes know what the form is for and when to complete it.
Save to create the form. You can keep editing it before you assign it.
Add Questions
Open the form and select type
Enter the question text.
Choose a question type.
For scale or numeric questions, set the minimum and maximum values.
For choice questions, enter each answer option.
Mark the question as required if athletes must answer it.
Repeat to add further questions. Reorder them if your platform supports drag to reorder.
Save your changes.
Assign to Athletes or a Group
Navigate to Assigning a form article for steps on this
Note: For how athletes complete an assigned form, see the companion article on completing check-ins.
Slider Question Guide
When to use
Only for answers that are genuinely numbers on a scale -effort ratings, counts, percentages. A slider always saves a number. Labels (words like "Low", "High", "10:15") are allowed and useful as visual guides along the track - but they are decoration only.
The stored answer will be the number the handle lands on, never the label text. If you need the words or a time format in your data, use a different question type.
Do:
Set Min, Max, and Step first, before adding any labels
Use word labels freely for context (e.g. "Low" at 0, "High" at 10) - just don't expect them in your report data
Keep every label's value within the Min–Max range
If a label ends up misplaced, fix it by editing the label's value, not by fiddling with the range
What not to do
Don't change Min, Max, or Step after your labels are set. Labels keep their original values, and changing the range underneath them knocks them off the track - they'll display outside the question and stay broken until each label's value is manually corrected to fit the new range. Get the range right first; if you must change it, re-check and re-fix every label afterwards.
Don't use a slider for times, durations, or categories - a label like "10:15" or "Poor / OK / Great" still saves as a plain number
Don't place labels outside the Min–Max range - they'll render outside the question box
Other Options
Times/durations -> Dropdown with the time options you want (saves exactly what's picked)
Categories or worded scales -> Radio group or Dropdown
Star/level style ratings -> Rating question
Rule of thumb: labels are for reading, numbers are for storing. Decide your number range first, decorate with labels last, and don't move the range once labels are on.
